2008 USAV Registration

All members (player, coach, staff) must register and pay for themselves ONLINE before tryouts at the Florida Region website.

All previous membership forms are invalid.

With online signature, the parent completing for a junior member authorizes she/he has legal capacity to act on behalf of the minor.

With online signature, you will be asked for their Driver’s License or State ID number, so have it on hand before beginning the process.

Club Directors, coaches and staff may NOT register a junior player unless they are the parent/guardian of that minor.

The registration site accepts credit cards. When you get to the payment information, select “pay now via credit card” and complete the required credit card information.

Player membership cards are to be printed at completion of registration and must be presented at tryouts in order to participate in tryouts.

If a player does not have a membership card at tryouts, they will not be permitted to tryout.

ALL coaches must register themselves individually BEFORE they can participate in tryouts and/or be in presence or interact with players. Coaches will need to pay for themselves at the time of registration. Coaches must print the membership card at registration and present to the club before tryouts.

Coaches should have a successful background check completed before being around players. Or, at minimum, be in a “pending” status for their background check.

Follow the instructions for either Returning Member or New Member. If you were registered with USAV last year you are considered a Returning Member.

If you are Returning and do not know your user name and password, request it online when you initially go to the registration site. The information will be sent to you via your email, once you receive it you may register.

Each registration page/screen is self-explanatory.

Be sure to select Gainesville Juniors on the club pull down menu on 2nd screen of the registration process.

Using your credit card, pay the registration amount. Keep your receipt.

Be sure to print the membership card at the end of registration. Membership cards must be presented at tryouts.
